This course provides a framework to assess individual barriers & support behavior change. We explore psychosocial issues that may be blocking participants from adopting healthier behaviors & provide strategies to identify & overcome these barriers. We conclude with a review of the major learning theories with a focus on empowerment.
Objectives:
- Discuss the chronic care model.
- Describe coping & individual assessment.
- List psychosocial, emotional & life barriers to diabetes self-care.
- Discuss strategies to assist people living with diabetes with individualized coping skills.
- Describe behavior change models & approaches.
Learning Outcome:
Diabetes team members will gain the skills and knowledge needed to identify coping strategies and provide individualized support for people with diabetes.
Target Audience:
This course is a knowledge-based activity designed for individuals or groups of diabetes professionals, including RNs, RDs/RDNs, Pharmacists, Nurse Practitioners, Clinical Nurse Specialists, Physician Assistants, and other healthcare providers interested in staying up to date on current practices of care for people with prediabetes, diabetes, and other related conditions. The practice areas for RDs/RDNs for CDR reporting are healthcare, preventative care, wellness, and, lifestyle along with, education and research.
Instructors: Beverly Thomassian RN, MPH, CDCES, BC-ADM is a working educator and a nationally recognized diabetes expert for over 25 years. She is an Associate Clinical Professor at UCSF and Founder of Diabetes Education Services.
