The landscape of metabolic monitoring has fundamentally shifted; the U.S. FDA authorizes the first Continuous Glucose-Ketone Monitor. The Libre Duo 10 Day Continuous Dual Glucose-Ketone Monitoring System is available for people aged 2 years and older living with diabetes. The Libre Duo 10 Day, the size of two stacked pennies, is the first wearable device in the U.S. that continuously monitors ketone levels, and the first in the world to continuously monitor both ketones and blood sugar (glucose) together in a single device.
For healthcare professionals, this provides a highly anticipated tool that shifts ketone surveillance from a reactive, point-in-time intervention to a proactive, continuous dataset.
“Today’s authorization is a breakthrough for the safety of children and adults living with diabetes,” said Michelle Tarver, M.D., Ph.D., Director of the U.S. Food and Drug Administration’s Center for Devices and Radiological Health. “Knowing that ketone levels are rising, and having that information in real time, around the clock, can be the difference between early intervention and a life-threatening emergency.”
From Single Point Urine Test to Continuous Ketone (& Glucose) Monitoring
For decades, clinicians and people living with diabetes have relied on single-point urine or blood finger-stick tests to measure beta-hydroxybutyrate. These traditional options are frequently underutilized due to the physical and economic burden of separate testing supplies, or because early symptoms of ketosis are mistakenly attributed to minor viral illnesses. By integrating continuous ketone monitoring (CKM) directly into a standard 10-day glucose sensor, the Libre Duo automatically tracks real-time trends and pushes wireless alerts to a patient’s smartphone when concentrations begin a dangerous upward trajectory.
This early warning window provides individuals, caregivers, and providers with the invaluable gift of time—allowing for oral hydration, supplemental insulin correction, or remote clinical guidance to abort the crisis before an emergency department visit or intensive care admission becomes mandatory.
Continuous background tracking acts as a vital “check engine light,” capturing a metabolic shift that continuous glucose monitoring (CGM) alone would entirely miss.
The primary clinical utility of this multi-analyte biowearable is the prevention of diabetes ketoacidosis (DKA), a life-threatening acute metabolic crisis.
Checking for Ketones is especially critical for individuals with:
- Type 1 Diabetes, with higher risk of DKA
- SGLT-2 Users (T1 & T2) to detect euglycemic DKA
- Ketosis Prone Type 2, prior DKA, higher ketone excursions
- Capillary ketones more than 0.8 mmol/L predict future DKA
Robust Clinical Evidence & Device Integration
Robust clinical evidence supports the FDA’s regulatory clearance. The approval was backed by data from six clinical studies involving over 600 participants. The trials demonstrated high accuracy across various physiological ranges and proved that the sensor could effectively flag clinically meaningful deviations in ketone levels well before the formal onset of DKA.
Looking ahead to clinical integration, this device marks a major step toward building safer, automated healthcare ecosystems. Abbott has established a new “integrated continuous glucose-ketone” (iCGK) standard, paving the way for immediate and future interoperability.
The manufacturer plans a U.S. commercial rollout later this year, with automated insulin delivery (AID) pump integrations expected to follow shortly.
Compatibility with Beta Bionics’ iLet and Sequel Med Tech’s twiist systems is slated by the end of this year, followed by Tandem, Insulet, and Medtronic integrations on the horizon.
As you prepare to counsel people with diabetes on this new technology, it is important to reinforce that ketone trends must always be interpreted alongside concurrent glucose data and physical presentation.
