Sarah Hormachea, MS, RD, CDCES, BC-ADM

Sarah is passionate about coaching and mentoring diabetes care and education professionals. She is known for her practical approach to translating complex health information and enjoys exploring the “how” and “why” behind the science of diabetes as much as the “what.”

Sarah provides individualized, one-on-one coaching for clinicians looking to strengthen their clinical skills or prepare for the CDCES or BC-ADM certification exams. Whether preparing for a first attempt or planning a second try, customized guidance and support can make a meaningful difference.

By day, Sarah is the owner of Sarah Hormachea: Diabetes Care and Education, LLC, where she provides services including DSME program and project management, medical nutrition therapy, telehealth integration, healthcare technology optimization, and consultative support for clinics and organizations seeking to strengthen their diabetes care programs.

By night, she advocates for the diabetes care and education profession. She serves on the Board of Directors for the Association of Diabetes Care & Education Specialists and was honored as Colorado’s 2022 Diabetes Educator of the Year.
